Composite Achievement across our 42 Partner Practices

The four Tables below summarize results on Better Health’s Composite Outcomes and Care Process Standards across all partner practices, first alphabetically (Table 2a) and then alphabetically within health care system (Table 2b). Table 2c sorts the practices by rank order of achievement on the Composite Outcome Standard; Table 2d sorts the practices by rank order of achievement on the Composite Care Process Standard. In these tables, practices whose measures are derived from their electronic medical records are separated from the results of practices whose measure4s are derived from paper-based records. As described in the Part Three Overview throughout the site-level reporting, we use stars to identify practice sites that fall within approximately the top decile (10%) of all practices and relevant practice groups by insurance type.

These data describe 25,634 patients – 23,101 from our 31 electronic medical records-based practices, and 2,533 from our four paper-based health systems.
