Spotlighting Outstanding Achievement and Improvement

We honor all practices that voluntarily participate in Better Health's measurement and reporting activities. Measurement matters, and it takes a commitment to excellence to publicly showcase quality scores. Improvement is tough work, particularly in caring for people with chronic disease, where day-to-day management relies largely on patients' behavior and adherence to medication regimens.

These 27 practices had quality scores in the top 10% of our measurement categories or improved most in their care of their diabetes patients in 2009. All have earned gold stars.
