Improvements on Most Standards from calendar 2007 to July 2007-June 2008

Figures 8-10 displays our regional achievement in two time periodsfor 30 EMR-based practices in 3 health systems that reported in two time periods: calendar 2007 (from our first Community Health Checkup) and July 1, 2007 through June 30, 2008 (theour current report). across the 30 electronic medical records-based practices in 3 health systems that reported in both of these time periods. Calendar 2007 data represents 23,461 patients of 30 practices in 3 systems, and 300 primary care physicians. July 1, 2007 – June 30, 2008 data represent 23,035 patients of the same 30 practices in 3 systems, and 286 primary care physicians. Figure 8 describes changes in our Composite Outcome and Care Process Standards; Figures 9 and 10 also display changes in our results for individual Outcome and Care Process standards.

Despite the short interval between Checkups, the current report describes unanticipated improvements in our overall composite Outcome and Process Standards, with each showing a slight but noticeable improvement in achievement. 



Figures 9 and 10 display our regional achievement on our individual Outcome and Care Process standards over the two reporting periods. Figure 9 displays the composite Outcomes Standard and its five individual standards, while Figure 10 provides analogous information for our Care Processes composite and four individual standards.

Despite the short interval between Checkups, achievement improved for the composite Outcomes standard and each of the individual Outcomes standards but the “weight control” and “non-smoking” standards. These highlight excellent targets for intensified efforts.

More patients also met the composite Care Process Standard and each of the four individual Care Process standards in the current reporting period, except for eye examinations, which remained stable.
