20% of patients with diabetes don't properly monitor their blood sugar levels. On day-to-day basis, high levels can cause tiredness, blurred vision and cause more frequent illness -- and increase the risk of serious complications.

 

Achievement in Commercially Insured Patients

The tables that follow provide information on individual practice sites’ achievement on Better Health’s Composite Outcome and Care Process Standards by insurance category. The practices that are reported in each table include those for which we have measures for at least 50 patients of the given insurance type; thus, the number of sites reported will vary by insurance type. In addition, as described earlier, because our sampling of sites with paper-based records does not enable us to provide a confident estimate of achievement by insurance groupings, these sites are not included in the tables.

We separately report relevant practices’ achievement for patients covered by Medicare, for Commercially insured patients, for Medicaid patients, and for uninsured patients.

The tables on this page describe the achievement among Commercially insured patients for our 28 electronic medical records-based practices who cared for at least 50 Commercially insured patients with diabetes in July 2007 - June 2008.  As before, exceptional achievement is highlighted by stars.
