Better Health Greater Cleveland

Better Health Greater Cleveland is an unparalleled alliance of regional stakeholders that is committed to improving the health and the quality of care for Greater Cleveland's residents with common chronic medical conditions.

We believe that everyone has a role to play to improve health and health care. We believe that we all can improve our performance. We are dedicated to sharing reliable information that can inform all health care stakeholders in Northeast Ohio.

Our Aims

  • Promote high-quality and equitable care for people with chronic medical conditions that are typically managed in primary care physicians' offices.
  • Report clinical measures of health outcomes and routine patient care of the region's primary care practices to motivate improvement.
  • Help people who live with chronic conditions to be more knowledgeable, active and confident in their care.
  • Implement strategies to help patients and physicians work together to make Greater Cleveland healthier.
  • Maximize recovery and minimize re-admissions by targeting selected elements of hospital care for quality improvement and improving transitions from the hospital to the community.

Funding for Better Health Greater Cleveland comesfrom contributions and grants that are unrestricted or dedicated to one or more of our program priority areas. We receive leadership, financial and in-kind support from the local community and from statewide and national organizations. Our partner organizations donate goods and services, and their volunteers contribute hundreds of hours working toward Better Health's goals.
